What you avoid matters, too

Healthy aging is shaped by what we practice, but also by what we avoid. Tobacco, alcohol, and other substances deserve a candid discussion without shame.

Substance use is easy to discuss badly. Moral language pushes people away, while wellness marketing can minimize real risk. I prefer a direct approach: name the evidence, recognize that dose and context matter, and make help easier to reach.

Avoiding tobacco is foundational. Alcohol is not required for health. Other substances can become more dangerous with age, changing health, or new medications even when the dose has not changed.

There is no healthy role for tobacco

Smoking remains a major preventable cause of cancer, cardiovascular disease, lung disease, and early death. Changing the delivery system does not make the exposure harmless. Vaping and smokeless products can maintain nicotine dependence even when the pattern of risk is different.

Alcohol is not required for health

Alcohol causes at least seven types of cancer. Older population studies sometimes made moderate drinking look protective for the heart, but those comparisons are tangled with differences in income, diet, social life, and prior health. If you do not drink, there is no health reason to begin.

The risk depends on more than the substance

Cannabis, sedatives, stimulants, opioids, and alcohol can affect sleep, mood, judgment, falls, driving, breathing, and medication safety. Dose and frequency matter. So do age, route, health history, and combinations. A substance that feels familiar can still become riskier when the context changes.

Help works better than shame

Medication treatment, counseling, quitlines, peer support, and harm-reduction strategies can all help. Dependence is a health condition, not a character flaw. Many people need more than one attempt, and that does not make the next attempt pointless.
